Can I request a specific item be cleaned differently?

Yes — that's the whole point of the home plan. Tell us what item, what level, what room. We update the plan and it applies to every visit.

The home plan is meant to be tuned. A standing item-specific request becomes part of the recurring template, not a per-visit instruction you have to remember to mention. If you want your antique desk skipped entirely, that's a skip-level on that item. If you want the master shower scrubbed harder than the guest, those are different levels on the same item type. If you have a fragile piece you want us to avoid touching, we mark it.
